Clinical Utility

Adenovirus is an important cause of morbidity and mortality in the transplant setting, causing pneumonia, hemorrhagic cystitis, hepatitis, encephalitis, pancreatitis, enteritis, and disseminated disease with the mortality rate reaching 60% in some especially high risk situations such as pediatric HSCT. Since proper management is dependent upon early diagnosis, quantitative adenovirus DNA PCR is useful for detecting the virus, tracking the course of infection, and monitoring response to treatment.

Procedure

Extraction of adenovirus DNA from plasma, urine, other biological fluids, or tissues followed by amplification and detection of known adenovirus strains using real-time, quantitative PCR. An internal control is added to ensure the extraction was performed correctly and the PCR reaction was not inhibited. Viracor-IBT's assay design includes multiple targets to account for viral mutations, which significantly reduces the chance of false negative results. This test has not been cleared or approved for diagnostic use by the U.S. Food and Drug Administration.

Specificity

Detects all 52 serotypes in one assay. The primers and probes used in this assay are specific for known adenovirus strains based on similarity search algorithms. Additionally, no cross reactivity was detected when tested against BKV, CMV, EBV, HSV-1, HSV-2, HHV-6 variant A, HHV-6 variant B, HHV-7, HHV-8, JCV, parvovirus B19, SV-40, and VZV.

Assay Range

  • 100 copies/mL to 1 x 1010 copies/mL (all specimen types except fecal, nasal swab, NP swab, tissue [Qual] and tissue [Quant]).
  • Fecal, nasal swab, NP swab and tissue [Qual] specimens report as qualitative (Detected/Not Detected).
  • Tissue [Quant] specimens report as 10 copies/mg to 1 x 109 copies/mg.

Causes For Rejection

Wood shafted swab, calcium alginate swab, specimens greater than 96 hrs old, or specimens other than those listed.

Ship Monday through Friday. Friday shipments must be labeled for Saturday delivery. All specimens must be labeled with patient's name and collection date. A Viracor-IBT test requisition form must accompany each specimen. Multiple tests can be run on one specimen. 7507 amniotic fluid - 2 mL collected in a sterile, screw top tube. Ship Monday thru Friday. Specimens shipped at ambient temperature must be received less than 48 hours from draw. Specimens greater than 48 hours from draw will be accepted if they have been stored frozen and shipped on dry ice overnight to Viracor-IBT.
7509 BAL - Test available for NY samples.  2 mL collected in a sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7504 bone marrow - 2 mL collected in EDTA tube. Ship at ambient temperature Monday thru Friday. Do not centrifuge. Specimen must be received within 96 hrs of collection.
7526 bronch wash - Test available for NY samples.  2 mL collected in a sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7503 CSF - Test available for NY samples.  2 mL collected in a sterile, screw top tube. Freeze and ship on dry ice Monday thru Friday. Specimen must be received within 96 hrs of collection.
7508 fecal - (CPT Code:87798 X 1) Collect small amount of fecal material (size of pea, or 2 mL liquid stool) and place into screw top tube for shipment. Store frozen and ship on dry ice for overnight delivery to Viracor-IBT.
7531 nasal asp - Test available for NY samples.  2 mL collected in a sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7530 nasal swab - (CPT Code:87798 X 1) Sterile swab placed in 2 mL sterile saline, M4, or viral transport media in a sterile, screw top tube. Do not use calcium alginate swab or wood shafted swab. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection
7513 nasal wash - Test available for NY samples.  2 mL collected in a sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7524 NP aspirate - Test available for NY samples.  2 mL collected in a sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7525 NP swab - (CPT Code:87798 X 1) Sterile swab placed in 2 mL sterile saline, M4, or viral transport media in a sterile, screw top tube. Do not use calcium alginate swab or wood shafted swab. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7547 NP wash - Test available for NY samples.  2 mL collected in a sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7512 pericardial fluid - 2 mL collected in a sterile, screw top tube. Ship Monday thru Friday. Specimens shipped at ambient temperature must be received less than 48 hours from draw. Specimens greater than 48 hours from draw will be accepted if they have been stored frozen and shipped on dry ice overnight to Viracor-IBT.
7501 plasma - Test available for NY samples.  Collect 4-5 mL whole blood in EDTA or ACD tube, centrifuge and transfer 2 mL plasma to sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7511 pleural fluid - 2 mL collected in a sterile, screw top tube. Ship Monday thru Friday. Specimens shipped at ambient temperature must be received less than 48 hours from draw. Specimens greater than 48 hours from draw will be accepted if they have been stored frozen and shipped on dry ice overnight to Viracor-IBT.
7510 serum - Collect 4-5 mL whole blood in red top tube, centrifuge and transfer 2 mL serum to sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7523 throat gargle - Test available for NY samples.  2 mL collected in a sterile container then transferred to sterile, screw top tube for shipment. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
7506 tissue [Qual] - (CPT Code:87798 X 1) Place fresh tissue in a sterile, screw top container. The preferred handling is to not add water, saline or other fluid media to the tissue container; however, fluid media filled containers will be accepted for qualitative results. Store frozen and ship on dry ice for overnight delivery to Viracor-IBT. Formalin fixed, paraffin embedded tissue will not be accepted.
7505 tissue [Quant] - Place a minimum of 5 mg fresh tissue (approximately ½ of a pencil eraser size) in a sterile, screw top container. Do not add water, saline or other fluid media to container. Store frozen and ship on dry ice for overnight delivery to Viracor-IBT. Formalin fixed, paraffin embedded tissue will not be accepted.
7519 trach asp - 2 mL collected in a sterile, screw top tube. Ship at ambient temperature Monday thru Friday. Specimen must be received within 96 hrs of collection.
